# **FORM UPDATE**

Notification of newly-created forms, modified documents and form deletions.

### **Opting out of the Ohio Form Removal**

DocMagic will be removing five state-specific forms from Ohio loan disclosure packages by April 1, 2018.

By opting out of this system default you are requesting to continue to receive forms whose requirements were removed by Ohio’s 2017 House Bill 199. These obsolete forms will be added to your account to continue to return for Ohio loans.
